Patients Over 75
If you are aged 75 years or over you will have a named GP within the practice.
That does not mean you will always see that GP, only that they will co-ordinate any care with the other clinicians.
You should be seen annually by your doctor, the practice nurse, district nurse or health visitor.
This can be arranged at the surgery or, if that is not possible, in your home.
